In this video, I will show you the reflex test to diagnose a tear of the lower posterior labrum. Hello! And welcome to Physiotutors. The presence or absence of pain during the reflex test was evaluated by K. et al. in 2004 as an indicator of success for conservative treatments for shoulder instability at the posterior-inferior level and to identify the pathological lesion responsible for the pain during the reflex test. They found that the test had 90% sensitivity and 85% specificity for detecting posterior-inferior labral lesions, and the presence of pain indicated a failure of conservative treatment. In our opinion, this test has high clinical value in practice. 

To perform the reflex test, position the patient sitting and position yourself behind them. While stabilizing the scapula with one hand, hold the affected arm at 90° of abduction and internal rotation. Then, apply a longitudinal force through the humerus at the elbow and move the arm into horizontal adduction. The test is positive with a sudden clunk as the head of the humerus slides out of the glenoid. In a painful reflex test, this coincides with a sharp pain. A second clunk may be observed when the arm is finally returned to the initial position and the head of the humerus returns to the glenoid. Okay!
